On or about April 3, 2017, the employer did not ensure employees were protected while smoothing out the dust in the box underneath the dust collector (Ultra Industries, Inc. Model #BB-49-84-IIIG, Serial #BB-4216) on the Y Processing Line in the Rubber department. An employee was leveling off dust with his hand in the box directly underneath the rotating airlock, resulting in a partial finger amputation.
