29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a.) On or about April 3, 2017, the employer did not ensure employees were protected while smoothing out the dust in the box underneath the dust collector (Ultra Industries, Inc. Model #BB-49-84-IIIG, Serial #BB-4216) on the Y Processing Line in the Rubber department. An employee was leveling off dust with his hand in the box directly underneath the rotating airlock, resulting in a partial finger amputation.
