29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: On or about October 2, 2019 at the Coreslab Structures Facility - Employees were exposed to serious eye injuries during Plasma cutting activities in the Plasma Cutting Area in that were observed using Plasma gun with reading glasses as form of eye protection.

29 CFR 1910.243(c)(1): Abrasive wheel(s) were used on portable grinder(s) which were not provided with safety guard(s) meeting the requirements specified in 29 CFR 1910.243(c)(1) through (c)(4)(a): On or about October 2, 2019 at the Coreslab Structures Facility - Employees were exposed to serious lacerations and or struck-by injuries, in that the Dewalt Portable Hand Grinder did not have a 180 degree guard that protected hands from the cutting wheel.
