On or about and prior to October 2, 2024, at the worksite located at 1615 West Arrow St, Marshall, Missouri 65340 employees were exposed to serious fall hazards while conducting material handling activities. Employees were using a portable ladder where the side rails did not extend at least 3 feet past the upper landing surface to access and egress an elevated work surface while rigging precast concrete parts to overhead gantry cranes.
