MARSHALL, MO 65340, the employer did not ensure that the Powered Industrial Truck Operators for the facility had a current certification to operate the equipment, in that the last performance evaluation was conducted on May 4, 2017 . Employees were exposed to serious injuries due to not conducting the revaluation per the instructions of the standard.
