29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:  At the commercial facility located at 1615 W Arrow Street, in Marshall Missouri 65340, on October 2, 2024, and at times prior: The employer did not establish a monitoring program for work areas or tasks where employee exposure equaled or exceeded the action level of 85 decibels measured on the A-scale. Employees were exposed to continuous noise levels higher than 85 dBA in the following areas:  a) Sandblasting - An employee was exposed to continuous noise level at 421.1% dose of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent sound level of approximately 100.3 dBA. The sampling was performed for 358 minutes during one shift.

Employees were exposed to continuous noise levels higher than 85 dBA in the following areas:  a) Sandblasting - An employee was exposed to continuous noise level at 421.1% dose of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent sound level of approximately 100.3 dBA. The sampling was performed for 358 minutes during one shift.

An employee was exposed to an 8-hour Time Weighted Average (TWA) of 98 ug/m3 of air which is 1.96 times the 8-hour TWA Permissible Exposure Level (PEL) of 50.0 micrograms per cubic meter of air while conducting sandblasting operations. Multiple deficiencies were identified in the employer's respiratory protection program, so that the program's effectiveness could not be assured.

29 CFR 1910.134(h)(1): Cleaning and disinfecting. The employer shall provide each respirator user with a respirator that is clean, sanitary, and in good working order. The employer shall ensure that respirators are cleaned and disinfected using the procedures in Appendix B-2 of this section, or procedures recommended by the respirator manufacturer, provided that such procedures are of equivalent effectiveness.   At the Coreslab Structures Inc. facility located at 1615 W Arrow Street in Marshall Missouri 65340, on October 2, 2024, and at times prior: The employer did not ensure that respirators were in good working order, as well as  cleaned and disinfected using the procedures in Appendix B-2 of 29 CFR 1910.134 or equivalent procedures recommended by the respirator manufacturer, provided such procedures or equivalent effectiveness. Respirators were visibly abraded and damaged. Dirt and oil residue was visible inside the employee's Bullard Free-Air sandblasting air supplied respirator hood. The employer had not inspected or tested respirators to evaluate their condition following extended use in abrasive blasting tasks.

Employees were exposed lung illnesses such as asthma and silicosis in that the respirators were not being stored and maintained to prevent damage and contamination.
