29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  At a jobsite located at 5841 Northwest 72nd Street, Kansas City, Missouri 64151, an employee was exposed to struck-by injuries. The employee was installing roof decking with a pneumatic nail gun without the use of eye and face protection.

29 CFR 1910.178(l): Operators were not trained in the safe operation of powered industrial trucks:  At a jobsite located at 5841 Northwest 72nd Street, Kansas City, Missouri 64151, employees were exposed to serious crushing and struck-by injuries. Forklift operators had not completed a competent training program. Training provided to the employees failed to meet the requirements of OSHA standard 1910.178(l) powered industrial trucks, operator training.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:  At a jobsite located at 5841 Northwest 72nd Street, Kansas City, Missouri 64151, employees were exposed to serious fall hazards. Employees who were installing roof decking was accessing the roof by climbing the open wall trusses. Employees were exposed to a fall hazard of 16 feet to the concrete floor below.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  At a jobsite located at 5841 Northwest 72nd Street, Kansas City, Missouri 64151, employees were exposed to serious fall hazards. Employees who were installing roof decking were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. The employees were exposed to a fall hazard of both 14 and 16 feet to the ground below.   RF Construction Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1342702, citation number 1, item number 1 and was affirmed as a final order on January 14, 2019, with respect to a workplace located at 4140 Nolan Road, Independence, Missouri 64055.  RF Construction Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1174782, citation number 1, item number 1 and was affirmed as a final order on October 4, 2016, with respect to a workplace located at 5241 North Antioch Road, Kansas City, Missouri 64119.
