29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or electrical shock and burns:  At the jobsite located at 18077 Hauser St, Overland Park, KS 66210:  Employees engaged in ground hand activities working below employees engaged in roofing activities were exposed to struck-by hazards in that head protection was not provided.

29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  At a jobsite located at 18077 Hauser St Lot #102, Overland Park, KS 66210, an employee was exposed to struck-by injuries. The employee was installing roof decking with a pneumatic nail gun without the use of eye and face protection  RF Construction Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1443902, citation number 1, item number 1 and was affirmed as a final order on January 15, 2020, with respect to a workplace located at at 5841 Northwest 72nd Street, Kansas City, Missouri 64151.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a)  At a jobsite located at 18077 Hauser St LOT # 102, Overland Park, KS 66210 employees conducting residential framing activities were exposed to serious fall hazards of approximately 11 ft from the ceiling joists to the ground below in that fall protection was not provided.  RF Construction Services  was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1479854, citation number 2, item number 2 and was affirmed as a final order on June 16, 2020, with respect to a workplace located at  8900 Dunraven St, Lenexa, Kansas.    RF Construction Services  was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1443902, citation number 1, item number 1 and was affirmed as a final order on January 15, 2020, with respect to a workplace located at 5841 Northwest 72nd Street, Kansas City, Missouri 64151.  RF Construction Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1342702, citation number 1, item number 1 and was affirmed as a final order on January 14, 2019, with respect to a workplace located at 4140 Nolan Road, Independence, Missouri 64055.  RF Construction Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1174782, citation number 1, item number 1 and was affirmed as a final order on October 4, 2016, with respect to a workplace located at 5241 North Antioch Road, Kansas City, Missouri 64119.
