The employer did not provide the Occupational Safety and Health Administration certification of abatement as required for Citation #1, Item #1 and Citation #2, Item #2 issued July 20, 2020.

The employer did not provide the Occupational Safety and Health Administration certification and documentation of abatement as required for Citation #2, Item #1 issued July 20, 2020.

29 CFR 1926.451(c)(2)(v): Fork-lifts shall not be used to support scaffold platforms unless the entire platform is attached to the fork and the fork-lift is not moved horizontally while the platform is occupied.  At a jobsite located at 8900 Dunraven St. in Lenexa, Kansas employees engaged in residential framing activities were exposed fall hazards and crushing hazards. Employees were exposed in that the scaffold platform was not attached to the forks or forklift.

29 CFR 1910.178(l): Operators were not trained in the safe operation of powered industrial trucks:  At a jobsite located at 8900 Dunraven St. Lenexa, Kansas employees engaged in framing activities were exposed to serious crushing and struck-by injuries. Forklift operator had not completed a forklift training program that met the requirements of OSHA standard 1910.178(l) powered industrial trucks, operator training.  RF Construction Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(l), which was contained in OSHA inspection number 1443902, citation number 1, item number 2 and was affirmed as a final order on 01/15/20, with respect to a workplace located at 5841 Northwest 72nd Street, Kansas City, Missouri 64151.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a)    At a jobsite located at 8900 Dunraven St, Lenexa, Kansas employees conducting residential framing activities were exposed to serious fall hazards of approximately 13 feet from the ceiling joists to the ground below in that fall protection was not provided.    RF Construction Services  was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1443902, citation number 1, item number 1 and was affirmed as a final order on January 15, 2020, with respect to a workplace located at 5841 Northwest 72nd Street, Kansas City, Missouri 64151.  RF Construction Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1342702, citation number 1, item number 1 and was affirmed as a final order on January 14, 2019, with respect to a workplace located at 4140 Nolan Road, Independence, Missouri 64055.  RF Construction Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1174782, citation number 1, item number 1 and was affirmed as a final order on October 4, 2016, with respect to a workplace located at 5241 North Antioch Road, Kansas City, Missouri 64119.
