29 CFR 1910.107(b)(5)(ii): Discarded filter pads and filter rolls were not immediately removed to a safe, well-detached location or placed in a water-filled metal container and disposed of at the close of the day's operation when not maintained completely in water:        On or about November 14, 2019, the employer failed to ensure that filter pads used during two-part spray finishing operations at the facility were properly discarded. Used filters that contained residual/leftover material from a category 3 flammable /combustible chemical (Polylite 33282-07: Styrene/resin and fiber glass mixture) were thrown in a garbage can outside of the facility.

29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment:    On or about November 14, 2019, the employer failed to ensure that several of the company's junction boxes, located  on the southwest wall next to the updraft paint booth were effectively closed. The 110/220/480 volts, 3 phase junction boxes were missing a knockout cover and thus had the potential to expose employees to electrical hazards from the wiring contained within the unit.

29 CFR 1910.303(g)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures, or by any of the following means identified in sub-paragraphs (A)through (D) of this paragraph:        On or about November 14, 2019, the 110/220/480 volts, 3 phase electrical junction box located on the southwest wall of the facility adjacent to the company's spray booth was missing the panel door. Employees tasked with completing a two part spray finishing application within the immediate vicinity of the spray booth were not protected from electrical hazards should over spray, dust or other material inadvertently contact the live parts of the electrical equipment.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:       On or about November 14, 2019, adequate strain relief was not provided for the rigid conduit that fed into the 110/220/480 volts, 3 phase electrical panel, located on the southwest wall of the production area; adjacent to the updraft spray booth. The conduit fitting and/or connector used for routing the electrical wires into the panel box was not attached thus potentially exposing workers who frequent the area to electrical hazards should the wiring be damaged and/or ripped out.
