29 CFR  1910.157(g)(4): Employees who have been designated to use fire fighting equipment as a part of the emergency action plan were not provided training in the use of appropriate equipment upon initial assignment and at least annually thereafter:   On or about July 11, 2023, Zehrco-Giancola Composites Inc. at the location of 1501 West 47th St. Ashtabula, OH 44004, did not ensure that supervisors designated to use fire fighting equipment as a part of the emergency action plan, were provided training in the use of appropriate equipment at least annually.

29 CFR  1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.   On or about July 11, 2023, Zehrco-Giancola Composites Inc. at the location of 1501 West 47th St. Ashtabula, OH 44004, mixers were handling hazardous chemicals to include but not limited to Styrene, Ethyl Acetate and Organic Peroxides during mixing operations without the benefit of a labeling system to identify the product and the associated hazards. The storage tank and chemical dispensers were mislabeled and/or lacked the appropriate hazard communication information.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  On or about July 11, 2023, Zehrco-Giancola Composites Inc. at the location of 1501 West 47th St. Ashtabula, OH 44004, did not provide the information contained in Appendix D to 29 CFR 1910.134 when providing filtering full and half facepiece respirators to be worn on a voluntary basis for the employees within the Mixing room.  Zehrco-Giancola Composites Inc., was previously cited for a violation of this occupational safety and health 29 CFR 1910.134(c)(2)(i), which was contained in OSHA inspection 1465693, Citation 1, Item 1 and was affirmed as a final order on May 26, 2020, with respect to a workplace located at 4212 Ann Ave., Ashtabula, OH 44004.

29 CFR  1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:  On or about July 11, 2023, Zehrco-Giancola Composites Inc. at the location of 1501 West 47th St. Ashtabula, OH 44004, failed to establish and implement those elements of a written program necessary to ensure that employees using respirators voluntarily were medically able to use that respirator. Employees using a tight fitting full and or half-face respirator on a voluntary basis when handling and mixing hazardous chemicals were not provided a medical evaluation.  Zehrco-Giancola Composites Inc., was previously cited for a violation of this occupational safety and health CFR  1910.134(c)(2)(ii), which was contained in OSHA inspection 1468639, Citation 1, Item 2a and was affirmed as a final order on August 19, 2020, with respect to a workplace located at 1501 West 47th St. Ashtabula, OH 44004.
