29 CFR 1910.29(b)(2): The employer did not ensure that midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent intermediate members are installed between the walking-working surface and the top edge of the guardrail system as follows when there is not a wall or parapet that is at least 21 inches (53 cm) high:  On or about March 6, 2020, the employer failed to ensure that a defective midrail, which was located on the maintenance access catwalk behind press #2, which was approximately 12 feet high, was repaired in order to protect employees from a fall from height hazard.  Maintenance employees access this catwalk to perform servicing of equipment.

29 CFR 1910.147(c)(2)(ii): If an energy isolating device is capable of being locked out, the employer's energy control program under paragraph (c)(1) of this section shall utilize lockout, unless the employer can demonstrate that the utilization of a tagout system will provide full employee protection as set forth in paragraph (c)(3) of this section.  On or about March 6, 2020, the employer failed to ensure that the hydraulic press, including but not limited to Press #56, was fully locked and tagged out prior to performing various tasks, such as but not limited to, maintenance and repair work.

29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy including steps A-D:  On or about March 6, 2020, the employer failed to develop and implement all of the steps necessary (steps A-D) to prepare for total isolation of all energy sources and then safe reenergization of such prior to performing various tasks, such as but not limited to, maintenance and repair work.
