29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:        On or about November 26, 2019, an employee was exposed to amputation hazards while performing the servicing/maintenance task of cleaning the Anhydrous Chute associated with the Anhydrous Centrifuge, and the employer did not develop, document, and utilize an energy control procedure for the Anhydrous Centrifuge.  This energy control procedure would include the following:      a.         A specific statement of the intended use of the procedure,  b.         Specific procedural steps for shutting down, isolating, blocking and securing machine or equipment to control hazardous energy,   c.         Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them, and  d.         Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.       VISCOFAN USA INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1136245, CITATION NUMBER 1, ITEM NUMBER 2a AND WAS AFFIRMED AS A FINAL ORDER ON JULY 21, 2016, WITH RESPECT TO A WORKPLACE LOCATED AT 50 COUNTRY CT, MONTGOMERY, ALABAMA 36105.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:        On or about November 26, 2019, an employee was exposed to amputation hazards while performing the servicing/maintenance task of cleaning the Anhydrous Chute associated with the Anhydrous Centrifuge, and the employer did not provide the employee with authorized employee lockout training.  This training would include:     a.         Recognition of applicable hazardous energy sources,   b.         Type and magnitude of the energy available in the workplace, and  c.         Methods and means necessary for energy isolation and control.       VISCOFAN USA INC.
