29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: A) Newman Technology, Inc., located at 100 Cairns Road, Mansfield, OH 44903: On or about December 5, 2019, the employer did not adequately train employees to lockout/tagout equipment such as, but not limited to, the Flex Tube Comp (Flex 30) machine (Model # MS14002-5) located in the 1215 Flex Tube Assembly Department. Employees did not control all hazardous energy sources such as, but not limited to, electrical and pneumatic before removing parts that were not ejected onto the chute. Employees were exposed to struck-by and pinch point hazards that could result in serious injuries. B) Newman Technology, Inc., located at 100 Cairns Road, Mansfield, OH 44903: On or about December 5, 2019, the employer did not adequately train employees to lockout/tagout equipment such as, but not limited to, the Flex Tube Comp (Flex 30) machine (Model # MS14002-5) located in the 1215 Flex Tube Assembly Department. Employees did not power down the equipment using established procedures and did not verify that all hazardous energy sources such as, but not limited to, electrical were isolated before cleaning inside the machine. Employees were exposed to struck-by and pinch point hazards that could result in serious injuries.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A) Newman Technology, Inc., located at 100 Cairns Road, Mansfield, OH 44903: On or about December 5, 2019, the employer did not ensure that all the steps outlined in 29 CFR 1910.147(d), required for the control of the hazardous energy sources, were performed. Employees did not control all hazardous energy sources such as, but not limited to, electrical and pneumatic on the Flex Tube Comp (Flex 30) machine (Model # MS14002-5), located in the 1215 Flex Tube Assembly Department, before removing parts that were not ejected onto the chute. Employees were exposed to struck-by and pinch point hazards that could result in serious injuries. Employees were exposed to pinch point hazards that could result in amputations.
