29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):(a) a.) On or about September 22, 2022, the employer failed to ensure employees implemented established procedures for the application of energy control, or effective alternative protection to ensure safe operations, while operating TBA Heat Guard/Interlock Flare Machine. An employee was attempting to remove excess lubricant from an unguarded hydraulic cylinder, due to a malfunction of the machine, when his right index finger became caught, resulting in an amputation requiring hospitalization.
