(b) Hammond Machine Builders Grinder / Polisher: On or about February 24, 2020, sufficient point of operation barrier guarding over the buffing / grinding wheel, and over the abrasive belt grinding head was not provided to protect operator and other employees working in the area from ingoing nip points, and rotating parts.     (c) Hammond Machine builders Grinder / Polisher: On or about June 11, 2020, sufficient point of operation barrier guarding over the buffing / grinding wheel, and over abrasive belt grinding head was not provided to protect operator and other employees working in the area from ingoing nip points, and rotating parts. 29 CFR 1910.219(c)(4)(i): Projecting shaft ends did not present a smooth edge and end, and projected more than one-half the diameter of the shaft without being guarded by nonrotating caps or safety sleeves:    (a) Devine Machine Grinder / Polisher: On or about June 11, 2020, an exposed  two (2) Inch diameter rotating shaft end located approximately 32 inches above the floor projected approximately two (2) inches, and was not guarded. This exposed employees to cuts, lacerations, and contusions.      (b) Hammond Machine Tool Company Grinder / Polisher: On or about June 11, 2020, an exposed two (2) Inch diameter rotating shaft end located approximately 32 inches  above the floor projected approximately two (2) inches, and was not guarded. This exposed employees to cuts, lacerations, and contusions.

29 CFR 1910.303(g)(1)(i)(B): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the width of the working space in front of electrical equipment was less than the width of the equipment or 762 mm (30 inches), whichever was greater :  (a) On or about June 11, 2020, in the south grind and polish area, the working space around, and in front of the two (2) electrical disconnect switch boxes were blocked from access by ductwork, boxes, worktables, and the grind and polish machine. This condition did not allow sufficient entrance, and access to the two 480 volt electrical panels/disconnects.
