29 CFR  1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  (a)  On or about June 21, 2023, employees operating the Cincinnati Press Brake (Machine No. 215,  & Serial No. 45140) in the Sheet Metal/Forming Department were exposed to a crushing and amputation hazard while running the deep ice chests. The employer did not ensure the point of operation was properly guarded, in-that the deep ice chest are run on the side of the press brake and the light curtains are disabled.
