29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) On or about January 19, 2024, employees operating the Cincinnati Autoshape hydraulic press brake were exposed to amputation hazards when the laser presence sensing devices were deactivated. (b) On or about January 19, 2024, employees operating the LVD (Lefebvre, Van Neste and Dewulf) hydraulic press brake were exposed to amputation hazards when the laser presence sensing devices were deactivated. (c) On or about January 19, 2024, employees operating the LVD (Lefebvre, Van Neste and Dewulf) PPEB hydraulic press brake were exposed to amputation hazards when the laser presence sensing devices were deactivated.
