29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On February 22, 2020, and at times prior, the employer exposed employees to caught-in and amputation hazards in that the employer did not develop an energy control procedure for the Litzler single end treater as employees engaged in maintenance and servicing activities (such as unjamming) on the machine.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a) On February 22, and at times prior, the employer exposed employees to caught-in and amputation hazards in that employees who engaged in untangling and realigning the tire cords being processed by the Litzler single end treating unit did not receive adequate training to identify and isolate the machines hazardous energy sources.
