29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about March 9, 2020, the employer did not ensure employees setting up, adjusting, and making blade changes to the Hyd-Mech H-22A horizontal band saw performed and followed the established energy control procedures by; ensuring the machine was turned off [per 1910.147(d)(2) requirements], isolating the machine from the energy source(s) [(d)(3)], affixing a lockout device [(d)(4)(i)], and verifying de-energization and isolation had been accomplished [(d)(6)], prior to adjusting the machine.
