On 5/20/2020, at the Murrayville facility, in the Mechanical Debone Department, an Authorized employee did not utilize proper lockout to the floor vacuum valve mechanism and incurred a thumb tip amputation. The employee had not been trained in the specific written procedures for the floor vacuum.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation. On 5/20/2020, at the Murrayville facility, in the Mechanical Debone Department, an Authorized employee routinely provided service and maintenance to a floor and shell vacuum that the employer did not provide training on the specific written lockout procedures for that equipment. The employee did not utilize proper lockout to the floor vacuum valve mechanism and incurred a thumb tip amputation.
