29 CFR 1926.150(a)(4): All fire fighting equipment was not periodically inspected and maintained in operating condition:  a) On or about 07/01/20, during the roof installation at the Jamestown Container facility located at 82 Edwards Deming Drive in Rochester, New York, in an area where flammable sealants were in use and ignition sources were present such as generators and air compressor, the employer did not remove from service a fire extinguisher that had been discharged and no longer operable, employees were potentially exposed to fire hazards.  NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  a)  On or before 06/30/20, at the Jamestown Containers facility, located at 82 Edward Deming Drive, in Rochester, New York, the employer did not prevent an employee from falling 30 feet to a lower level through a roof opening by covering the hole, barricading the hole or providing the employee with a personal fall arrest system.  ABATEMENT DOCUMENTATION REQUIRED
