29 CFR 1926.502(i)(4): All covers were not color coded or marked with the word "HOLE" or "COVER" to provide warning of the hazard: a. On or about July 25, 2020, employees were exposed to a 15 foot fall hazard though a roof access hatch which was 30 inches by 36 inches, in that, the cover over the hatch access consisted of debris such as insulation and loose plywood and was not color coded and had no marking to indicate that it was a cover for the access hatch to warn employees of the fall hazard.
