29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: a) At the jobsite located at 13290 Corbel Circle in Fort Myers as observed on or about 6 October 2020. Employees were exposed to an electrocution hazard in that the employees were using electrical tools supplied power through: 1: A defective Ground Fault Circuit Interrupter outlet that was damaged and could not be tested. 2: The extension cord in use for the employees working on the roof was missing the path to ground. 3: The extension cord in use had a cut in the outer insulation approximately 3 inches from the plug end exposing the energized hot conductor.
