Workers were wearing Personal Fall Arrest Systems and were not tied down to an anchor plate with lanyard.  The workers were exposed to a fall hazard.  CROSSROADS ROOFING & CONTRACTING was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1220328, citation number 1, item number 1, issued on 4/2/2017, and was affirmed as a final order on 8/18/2017, with respect to a workplace located at 372 Shadow Pines Drive, Wentzville, MO 63385.
