29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. On or about 4/14/2021, employees from Crossroads Roofing & Contractors, LLC were repairing a roof of a residential building located at 1605 N. 16th Street in St. Louis, MO. The ladder the employees were using to access the roof was not three feet beyond the upper landing or edge of the roof.
