The trench was composed of loose granular soil and was showing signs of fissures. The competent persons permitted the employee to work inside the trench with the hazardous conditions present and without an adequate protective system. The employee was engulfed and suffered bruises on his left hip, rib cage, and shoulder.  Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Certification of Corrective Action Worksheet").
