29 CFR 1926.651(j)(2):Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. a) 18 N Corona St, Colorado Springs CO 80903 : On March 25th, 2021 and at times prior, employees were exposed to cave-in hazards while installing a storm drain at a depth of approximately 9 feet, with a spoil pile directly next to the opening of the trench, sloughing materials outside cave-in protection, the soil being class c type soil, and an active roadway by the trench.

29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a) 18 N Corona St, Colorado Springs CO 80903 : On March 25th, 2021 and at times prior, employees were exposed to cave-in hazards while entering and exiting the trench to install a storm drain in an unprotected trench at a depth of 9 feet with two vertical walls. The competent person failed to remove employees from the hazardous location. Native Sun Materials Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.652(a)(1), which was contained in OSHA inspection number 1486881, citation number 1, item number 2 and was affirmed as a final order on December 2nd, 2020, with respect to a workplace located at 109 Silbani Lane, Florissant CO, 80816. Native Sun Materials Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.652(a)(1), which was contained in OSHA inspection number 1411342, citation number 1, item number 1-b and was affirmed as a final order on July 17th, 2019, with respect to a workplace located at Fountain Mesa Rd, Fountain CO, 80817.
