29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: Southwest side of the intersection of 3rd Ave. & Coffman St., East of 703 3rd Ave. #103, Longmont CO: On or about July 24, 2025, employees working in a vertical trench 6 feet in depth were not provided a safe way to exit the trench. Equipment such as but not limited to a ladder for entering or exiting the trench, was not readily available.

29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: Southwest side of the intersection of 3rd Ave. & Coffman St., East of 703 3rd Ave. #103, Longmont CO: On or about July 24, 2025, the employer failed to ensure that a competent person performs daily inspections of excavations, the adjacent areas, and protective systems, as demonstrated. This condition exposed employees to cave-in hazards and to the hazards of being struck by falling materials.

29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): Southwest side of the intersection of 3rd Ave. & Coffman St., East of 703 3rd Ave. #103, Longmont CO: On or about July 24, 2025, the employer failed to assure effective protection against cave in hazards while conducting trenching work. Employees were exposed to cave-in hazards while in vertical trench that measured to be approximately 6 feet deep without any protection.
