Bottom Gang Rip Saw Model SL-10 Serial #63. The employer failed to develop, document and utilize procedures to effectively control hazardous energy for employees that clean the rip saw. One employee suffered injuries which resulted in a finger amputation when performing cleaning operations on the piece of equipment which was thought to be de-energized.

One employee suffered injuries which resulted in a finger amputation when performing cleaning operations on the piece of equipment which was thought to be de-energized.

Bottom Gang Rip Saw Model SL-10 Serial #63. The employer failed to report the amputation within 24 hours to OSHA. The amputation was reported on August 11, 2020.
