29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) Western Bee Supplies, Inc.: On or about May 24, 2024 and at times prior, employees were exposed to caught in hazards from the unexpected energization of equipment while clearing jammed boards from the Body Machine Sizer's hopper and saw chain area without following orderly shutdown to ensure de-energization.
