29 CFR 1926.20(b)(1): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.  a) Alfredo Sanchez, at a jobsite located at Lot 127, Pickerington, Ohio 43147: On and/or preceding October 6, 2020, the employer failed to initiate and maintain a safety and health program, such as but not limited to an electrical safety program and a fall protection program, for employees conducting framing work.

The path to ground from circuits, equipment, and enclosures shall be permanent and continuous.   a) Alfredo Sanchez, at a jobsite located at Lot 127, Pickerington, Ohio 43147: On and/or preceding October 6, 2020, the employer did not ensure employees who used extension cords were protected from electrical contact hazards. Framers were using a yellow extension cord with the ground prong missing from the male end of a cord, which exposed the employees to a 125 volt electrical contact hazard.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   a) Alfredo Sanchez, at a jobsite located at Lot 127, Pickerington, Ohio 43147: On and/or preceding October 6, 2020, the employer did not ensure employees were protected from falls while engaged in framing activities. Employees, without fall protection, were conducting framing and sheathing activities on a two story house, thereby exposed to fall hazards of up to 20 feet.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  a) Alfredo Sanchez, at a jobsite located at Lot 127, Pickerington, Ohio 43147: On and/or preceding October 6, 2020, the employer did not ensure employees were trained on the recognition of fall hazards or the procedures required to be followed to minimize the fall hazards. Employees, without fall protection training, were conducting framing and sheathing activities while working from the roof of a two story house, thereby exposed to fall hazards of up to 20 feet.
