29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii): a. On or about October 20, 2020, at the new residential home, an employee installing roof shingles on a 7:12 pitch roof was not properly trained in the use of personal fall arrest systems (PFAS). The body harness was loose on the employee, and the retractable lanyard was attached to a positioning ring on the right-side instead to the D-ring on the back, exposing the employee to a serious injury in the event of a fall.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: a. On or about October 20, 2020, at the front of the new residential home, two employees were installing roof shingles with pneumatic nail guns without wearing safety glasses, exposing to eye injuries from flying particles/debris. The Mediterranean Construction & Roofing, Inc was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1220030, citation number 1, item number 1 and was affirmed as a final order on July 14, 2017, with respect to a workplace located at 236 Spanish Creek Dr., Lot 388, Ponte Vedra, FL 32081.
