29 CFR 1926.25(a):During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: New townhome construction located on Lot #59 at 386 Concord Street, Alpharetta, GA, Rear side: On or about 10/22/2020, the employer exposed employees to trip hazards in work areas located between two townhome buildings, in that the ground was not kept free of debris which included sheathing, framing lumber, and fire wall construction materials. A means to remove trash or discard construction debris was not provided by the employer.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: New townhome construction located on Lot #59 at 386 Concord Street, Alpharetta, GA, Second level, rear side: On or about 10/22/2020, the employer exposed employees to fall hazards of approximately 15-feet, 8-inches from the lower ground level, in that a fall protection system was not provided to employees engaged in framing and sheathing activities.
