29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:(a) (LOCATION) (IDENTIFY EQUIPMENT AND CONDITION(S)) (STATE HAZARD(S) TO EMPLOYEE(S)) On or about November 17, 2020, employees engaged in drilling operations were exposed to electrical shock hazards due to having unused openings in the following instances: a) Circuit breaker box in the parts room had a circuit breaker missing. b) Circuit breaker box located outside of night crew's trailer had a knockout missing.
