On or about July 6, 2025, on the south side of the job site; employees engaged in removal and maintenance of a pulsation dampener. Despite exercising all reasonable diligence to relieve all potentially hazardous stored energy from the pulsation dampener, the employer neither knew nor could have known that all of the stored energy had not been released.
