29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: On or about April 27, 2022, at the Independence Contract Drilling Rig #206 doghouse, two relocatable power taps were not used or installed in accordance with instructions included in the listing or labeling. One relocatable power tap was plugged into another relocatable power tap. This condition exposed employees to fire and electrical shock hazards.
