29 CFR 1910.106(b)(6): In locations where flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition. Sources of ingition may include open flames, lightning, smoking, hot sirfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat: On or about October 16, 2024, on the northwest corner of the drill site, employees utilized a fuel transfer pump used to connect to the unprotected terminals of the battery (source of ignition) while positioned directly on top of the fuel tank while diesel fuel was dispensed.
