DBI, Inc. dba Brookville Glove Manufacturer - On or about 12/9/20, and at times prior thereto, the glove turner machine did not have a guard to prevent employees hands from entering the area where the moving metal fingers inserted into the metal holes to turn the gloves. b.) DBI, Inc. dba Brookville Glove Manufacturer - On or about 12/9/20, and at times prior thereto, the band cutter did not have a guard installed over the cutting blades to prevent employees from coming in contact with the rolling blades. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
