29 CFR 1910.147(c)(1): Energy control program. The employer did not establish a program consisting of energy control procedures, employee training, and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup, or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. (a) On or about February 2, 2021, and at times prior, employees were exposed to amputation hazards in that an effective hazardous energy control was not implemented at the facility. The following deficiencies were observed: 1) Employees were performing a blade change on the HEM saw, model H90A, without de-energizing and isolating the machine, thus exposing them to amputation hazards in the event of an unexpected start-up. 2) When machines were locked/tagged out at times, employees performing the maintenance work are not applying their own locks/tags and are working under the lock/tags of a supervisor or facilities coordinator. This was observed when the employer attempted to correct the lack of de-energization/isolation for the blade change operation during the inspection. 3) Employees were not adequately trained on the proper implementation of the hazardous energy control program or procedures.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases, or vapors, or potentially injurious light radiation: (a) On or about February 2, 2021, the employer, Master Magnetics, Inc., did not ensure each exposed employee using the pressurized water/air hose to remove metal scraps from the metal magnet product, was protected from flying metal shavings, by use of eye protection.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: (a) On or about February 2, 2021, the employer, Master Magnetics, Inc., did not ensure safe access to the electrical panels was maintained. Panels were obstructed by a pallet containing forklift charging stations.
