29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a. On or about December 18, 2024, employees were exposed to hazards created by the unguarded point of operation on the Hoffman Brothers Strip Cutter XD-906Y machine, which was left unprotected while employees fed material through the guillotine-style blade.

On or about December 18, 2024, employees were exposed to electrical shock hazards while using a metal receptacle outlet box with knockouts used as a pendant at the Eagle Strip Cutter XD-906Y. The employer did not ensure that the electrical receptacle(s) provided were provided by an approved pendant.

29 CFR 1910.147(d)(3): Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). a. On or about December 18, 2024, an employee performed jam-clearing activities on an Eagle Strip Cutter XD-906Y machine while the equipment was energized and operational. The employer did not ensure that the cutter was disconnected from its electrical energy source prior to performing the servicing activity, thereby exposing the employee to caught-in hazards created from unexpected movement of the machine. Master Magnetics, Inc. was previously cited for a violation of its equivalent Occupational Safety and Health standard 29 CFR 1910.147(c)(1), which was contained in OSHA inspection number 1512903, citation number 1, item number 1, and was affirmed as a final order on April 28, 2021, with respect to a workplace located at 1211 Atchison Court, Castle Rock, CO 80109.
