29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  At the jobsites located at 15716 W 171st Terrace, Olathe, KS, employees engaged in residential framing activities were exposed to falls from approximately 11 feet from the roof to the ground below, in that the wood ladder used to access the roof did not extend 3 feet from the eave of the roof.

29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets:   At the jobsite located at 15716 W 171st Terrace, Olathe, KS, employees engaged in residential framing activities were exposed to struck-by hazards from falling tools or materials in that hard hats were not provided.  Miranda Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a), which was contained in OSHA inspection number 1153206, citation number 1, item number 1, and was affirmed as a final order on 07/20/16, with respect to a workplace located at 301-311 N. Liberty Drive, Independence, Missouri.

The Miranda Construction Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1394049, citation number 1, item number 1 and was affirmed as a final order on 07/18/19, with respect to a workplace located at 2300 Independence Ave., Kansas City, Missouri.
