29 CFR  1926.451(c)(2)(iv):Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:  Worksite located at 1180 NW Buckner Tarsney Road, Grain Valley, Missouri- Employee had climbed on to the top of a stack of plywood that was loaded on the forks of a front end loader that was not designed to hoist employees.

The employee could slip off the ladder or the ladder could fall over.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   Worksite located at 1180 NW Buckner Tarsney Road, Grain Valley, Missouri- Employees were observed using nail guns, hammers, and other tools and were not protecting their eyes with the use of any means of eye protection.  The Miranda Construction company was previously cited for a violation of this occupational safety and health standard or it's equivalent standard 1926.102(a), which was contained in OSHA inspection number 1394049, citation number 1, item number 1 and was affirmed as a final order on 7-18-2019, with respect to a workplace located at 2300 Independence Ave., Kansas City, MO 64106.  The Miranda Construction Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1523066, citation number 2, item number 2, and was affirmed as a final order on 8-31-21, with respect to a workplace located at 15716 W. 171st. Terrace, Olathe, KS 66062.

The Miranda Construction company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1394049, citation number 1, item number 2, and was affirmed as a final order on 7-18-2019, with respect to a workplace located at 2300 Independence Ave., Kansas City, MO 64106.
