29 CFR  1910.147(c)(1):Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  On or about May 7, 2021, the employer failed to develop and implement a written control of hazardous energy program which would include all of the requirements under the Standard.  Employees are exposed to energy sources such as but not limited to electrical, hydraulic and pneumatic on equipment to include but not limited to the cut off saws and straightening press.

29 CFR  1910.212(a)(1): One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  On or about May 7, 2021, the employer failed to ensure that  employees were protected from struck by hazards when operating equipment such as but not limited to the Enerpac straightening press.  The die and tool rest on the hydraulic press had no guarding in place.

29 CFR  1910.215(b)(5): Surface grinders and cutting-off machines. The maximum angular exposure of the grinding wheel periphery and sides for safety guards used on cutting-off machines and on surface grinding machines which employ the wheel periphery shall not exceed 150 deg. This exposure shall begin at a point not less than 15 deg. below the horizontal plane of the wheel spindle. (See Figures O-12 and O-13)  On or about May 7, 2021, the employer failed to ensure that the angular exposure of the grinding cut off wheel periphery and sides was no greater than 150 degrees.   The degree of measurement at the time of inspection exceeds 150 degrees and is approximately 180 degrees.

29 CFR  1910.1200(f)(1): Labels on shipped containers. The chemical manufacturer, importer, or distributor shall ensure that each container of hazardous chemicals leaving the workplace is labeled, tagged, or marked. Hazards not otherwise classified do not have to be addressed on the container. Where the chemical manufacturer or importer is required to label, tag or mark the following information shall be provided:   On or about May 7, 2021, the employer failed to ensure that all employees were trained in hazard communication chemical safety in the workplace.  Employees work with various chemicals including but not limited to oils, lubricants and fuels.
