29 CFR 1910.132(f)(2):The employer did not require employees to demonstrate an understanding of the training specified in paragraph (f)(1) of this section, and the ability to use PPE properly, before being allowed to perform work requiring the use of PPE: Kitchen: Employees used Oasis 146 Multi-Quat sanitizer to conduct dish, pots and pan washing operations. The employer did not provide personal protective equipment (PPE) training specific to the hazardous chemical prior to the employees performing work. Employees were potentially exposed to burns and skin irritation.

The employer did not conduct a workplace hazard analysis with written certification specific to the physical and chemical hazards prior to employees starting the assigned tasks.
