Test voltages and the maximum intervals between tests were not in accordance with Table E-4 and Table E-5. On or about May 21, 2021 electrical protective equipment was not subjected to periodic electrical tests in accordance with table E-4 and table e-5, exposing employees to the hazards associated with electrical shock. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.416(a)(1):Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: On or about May 21, 2021 employees were not protected from energized electrical equipment while installing two new 480 volt circuits, exposing employees to the hazards associated with electrical shock and or an Arc flash/ blast. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about May 21, 2021 one employee was hospitalized after being burned by an electrical arc/ flash, while performing electrical construction activities. The employer failed to report the hospitalization within 24 hours, as notification was not reported until May 26, 2021. No abatement documentation or certification is required of this item.
