29 CFR 1926.451(f)(7): The scaffold was erected, moved, dismantled, or altered without the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling, or alteration: (a) General Masonry, LLC. at 1771 Pearl Street, Denver, CO 80210: On and before June 22, 2021 the creating and exposing employer did not ensure that a fabricated frame scaffold was erected with the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling, or alteration. Employees and the general contractor worked on the second level of the fabricated frame scaffold which was missing end-rails and mid-rails, both on the exterior and interior of the scaffold. This condition exposed employees and the general contractor to a fall of approximately 12 feet.

29 CFR 1926.451(g)(1): Each employee on a scaffold 10 feet above a lower level was not protected from falling to the lower level: (a) General Masonry, LLC. at 1771 Pearl Street, Denver, CO 80210: On and before June 22, 2021 the creating and exposing employer did not ensure that employees and the general contractor were protected from a fall from elevation while working on the second level of the fabricated scaffold in that end-rails and mid-rails, both on the exterior and interior of the scaffold, were not installed. This condition exposed employees and the general contractor to a fall of approximately 12 feet.
