29 CFR 1926.451(g)(4)(iii): Midrails, screens, mesh or equivalent structural members were not installed between the top edge of the guardrail system and the scaffold platform:  a) On February 27, 2026, employees were working from a tubular welded framed scaffold without the midrail installed between the top edge of the guardrail system and the scaffold platform.   No abatement certification or documentation is required for this item.

29 CFR  1926.451(g)(1): 29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level:   a) On February 27, 2026, employees performing masonry work from a tubular welded framed scaffolding were not protected from falling over approximately 25 feet along open sides and ends of the scaffold.  General Masonry LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1), which was contained in OSHA inspection number 1691115, citation number 1, item number 3 and was affirmed as a final order on January 24, 2024, with respect to a workplace located at 4861 N Hermitage Ave., Chicago, IL 60640.  No abatement certification or documentation is required for this item.
