At the dry dock area, next to the electrical power generator and an electrical power substation, a minimum of one regulated portable fire extinguisher was not provided or accessible to employees in case of a fire emergency.

29 CFR 1917.44(d): The employer did not direct motor vehicle operators to comply with posted speed limits and other traffic control signs or signals, and written traffic instructions: On or about 23 June 2021, at the main road, Sunbay Marina, Fajardo, PR. Employees engaged in marinas operations and civilians were exposed to struck by and caught by or in-between hazards as evidenced by the lack of traffic safety controls/information such as signs indicating pedestrian traffic clearly posted and rumble strips/surface indentations at appropriate locations to remind drivers of speed.

Employees engaged in fuel dispensing operations at the marina had been working with no approved life vests so as to protect them from injury.
