Throughout The Warehouse: The storage rack base plates were not anchored to the floor to prevent movement. Employees were exposed to struck-by and crushed-by hazards. b. In the 2-liter area: A damaged storage rack was used to store 2-liter beverage bottles, and other beverage products on pallets. Employees were exposed to struck-by and crushed-by hazards. c. In the cups and lids area: A storage rack with a damaged column was used to store boxes of cups, lids, straws and plastic beverage trays on pallets. Employees were exposed to struck-by and crushed-by hazards. d. In the cups and lids area: A storage rack with a damaged column was used to store boxes of cups, lids, straws and plastic beverage trays. All of the products and materials were on pallets. Employees were exposed to struck-by and crushed-by hazards. e.
