OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and/or crushed-by hazards. a. Warehouse: Racking system F-311-A was used with a damaged column and cross brace to store pallets of Gator-Aid. Employees were exposed to struck-by and crushed-by hazards. b. Warehouse: Racking system F-61-A was used with a damaged cross brace to store pallets of peanut butter and jelly. Employees were exposed to struck-by and crushed-by hazards. c. Warehouse: Racking system A-11-A was used with a damaged column to store pallets of 5-gallon bibs. Employees were exposed to struck-by and crushed-by hazards. d. Warehouse: Racking system C-41-A was used with a damaged column and cross brace to store pallets of 5-gallon bibs. Employees were exposed to struck-by and crushed-by hazards. e. Warehouse: Racking system A-21-A was used with a damaged column and cross brace to store pallets of 5-gallon bibs. Employees were exposed to struck-by and crushed-by hazards.
